1. Sponsor

The promotional drawing (“Drawing”) is sponsored and promoted by:

This promotion is offered solely by Beacon Psychiatry & Wellness PLLC. IntellxxDNA and the Tri-Cities Women’s Expo are not sponsors or administrators of this Drawing and are not responsible for its administration.

2. Eligibility

The Drawing is open to legal residents of Washington State who are 18 years of age or older at the time of entry.

Employees, contractors, and immediate household members of Beacon Psychiatry & Wellness PLLC are not eligible to enter.

Void where prohibited or restricted by law.

3. Drawing Period

The Drawing begins September 25, 2026 and ends October 25, 2026 at 11:59 PM Pacific Time.

Entries received after the closing time will not be eligible.

4. How to Enter

Eligible individuals may enter by completing an official Beacon Psychiatry & Wellness Drawing Entry Form during the promotional period.

No purchase, payment, appointment, consultation, or establishment of a patient-provider relationship is required to enter or win.

Limit one entry per person.

Consent to receive marketing or information about Beacon Psychiatry & Wellness services is optional and is not required to enter or win.

5. Prize

One (1) eligible winner will receive one Beacon Psychiatry & Wellness Precision Genomics Package with a retail value of up to $2,000.

The winner may select one of the following individual Precision Genomics packages:

• Medical Overview — $1,800 retail value

• Cognition & Memory — $2,000 retail value

• Mental Wellness — $2,000 retail value

• Women’s Health — $2,000 retail value

The selected package includes:

• The applicable IntellxxDNA genomic testing/report;

• Professional review, analysis, and interpretation of the results;

• One 60-minute results consultation; and

• One 45-minute follow-up consultation.

The maximum retail value of the prize is $2,000.

If the winner selects Medical Overview, the prize value is $1,800. The difference between the selected package’s retail value and the $2,000 maximum prize value will not be paid in cash, credited toward other services, or otherwise awarded.

The prize is non-transferable and has no cash redemption value. No cash or other Beacon services may be substituted for any unused portion of the prize.

6. Clinical Services and Scope of Practice

Winning the Drawing does not, by itself, establish a patient-provider relationship with Beacon Psychiatry & Wellness.

Before clinical services are provided, the winner must complete Beacon Psychiatry & Wellness’s ordinary intake, informed-consent, and other clinically appropriate requirements.

The winner must be physically located in Washington State at the time clinical services included in the prize are provided.

Genetic findings are not diagnoses or predictions of future health outcomes. Results will be interpreted in conjunction with the individual’s history, concerns, goals, and other clinically relevant information.

Recommendations may include nutrition, targeted supplements, lifestyle strategies, additional evaluation, laboratory testing, or other clinical considerations when appropriate.

All clinical services and recommendations will be provided within the professional scope of Beacon Psychiatry & Wellness and the treating clinician.

When a finding or healthcare need falls outside the clinician’s scope of practice, the winner may be referred to their primary care provider or another appropriate healthcare professional or specialist.

The prize does not guarantee any particular diagnosis, prescription, medication change, treatment recommendation, clinical result, or health outcome.

7. Winner Selection and Odds

One potential winner will be selected at random from all eligible entries received during the Drawing Period.

The random drawing is scheduled for October 26, 2026.

The odds of winning depend upon the total number of eligible entries received.

The winner does not need to be present at the drawing to win.

8. Winner Notification

The potential winner will be contacted using the telephone number and/or email address provided on the entry form.

The potential winner must respond within five (5) calendar days of the first notification attempt.

If the potential winner cannot be contacted, fails to respond within five calendar days, is determined to be ineligible, declines the prize, or cannot accept the prize in accordance with these Official Rules, Beacon Psychiatry & Wellness may select an alternate winner at random from the remaining eligible entries.

9. Acceptance of Prize

The selected winner must satisfy the eligibility requirements of these Official Rules before receiving the prize.

Acceptance of the prize does not obligate the winner to purchase additional testing, treatment, products, appointments, or services from Beacon Psychiatry & Wellness or any other provider.

Any additional services elected by the winner that are not expressly included in the prize are not part of the Drawing prize.

10. Privacy and Optional Contact

Information collected on the Drawing Entry Form will be used to administer the Drawing, verify eligibility, and contact the selected winner.

Participants may separately indicate that they would like information about Beacon Psychiatry & Wellness services.

Permission to receive information or marketing communications is optional and does not affect eligibility or odds of winning.

11. Winner Information and Publicity

Beacon Psychiatry & Wellness will not use the winner’s name, photograph, testimonial, health information, or other identifying information for advertising or promotional purposes without separate written authorization.

A list identifying the winner as permitted by applicable law may be requested after the Drawing by writing to:

12. Taxes

The winner is responsible for any applicable federal, state, or local taxes associated with acceptance of the prize. Any required taxes are payable only to the appropriate governmental authority and not to Beacon Psychiatry & Wellness.

13. General Conditions

By entering, participants agree to be bound by these Official Rules.

Beacon Psychiatry & Wellness reserves the right, subject to applicable law, to suspend, modify, or cancel the Drawing if fraud, technical problems, circumstances beyond its reasonable control, or other conditions compromise the integrity or proper administration of the promotion.

The Drawing will be conducted in accordance with applicable Washington law.
